- Recent photos of proprietors/authorized signatories
- Business address proof (rent/sale deed/utility bill)

- Declaration form confirming FSSAI compliance
- Certificate of Incorporation / Partnership Deed / Trust Deed
- GST Certificate (if applicable)
- Trade license / Shop & Establishment Certificate

- List of machinery and equipment

How to Apply Online via FoSCoS
- Visit FoSCoS Portal → Click “Apply for License/Registration”
- Select your state → Choose State License
- Fill Form B accurately: business name, address, turnover, food categories, authorized person
- Upload all documents in PDF/JPEG/PNG (max 2–5 MB)
- Pay application fee online
- Track status via application reference number
- Download FSSAI State License Certificate upon approval
